Immediate need
SUMMARY: Staff medical provider will provide services to the clients at CommWell Health, for treatment of HIV, Infectious Disease, Hep C and Prep Clinic working collaboratively with all disciplines within an integrative multidisciplinary environment.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provides direct outpatient care to patients.
- Prescribes medication utilizing clinic formulary for outpatients as feasible.
- Requisitions on and off-site lab and x-ray exams as needed.
- Participates in staff conferences to devise individual patient care plans, to revise and update clinic services, continuing quality improvement program and continuing education as appropriate.
- Participates in other related activities affecting the clinic program.
- Work with interdisciplinary teams through the organization to develop and maintain program policies and procedures and identify and eliminate barriers to integrated care.
- Participate in ongoing continuing education and maintain current knowledge of HIV/AIDS and related information by attending trainings, in services, attending professional development events.
- Develop and maintain required records, reports, and statistical data.
- Participate in all Patient Centered Medical Home quality efforts and attend required meetings.
CERTIFICATES, LICENSES, REGISTRATIONS:
Current NC licensure
Prefer active American Academy HIV certification
