Responsibilities may include:
•Conducting a variety of home visits with patients including start of care, routine, recertification and resumption of care.
•Providing education to patients and their families.
•Utilizing our electronic medical record system, Brightree, to complete documentation including OASIS.
•Maintaining current patient records and progress notes.
•Developing and establishing a plan of care with physician.
•Supervising LPNs and Home Health Aides and document delivery of care as required.
Requirements for this position include:
•Minimum of an associate’s degree or equivalent in Nursing from an accredited nursing program.
•RN license.
•Nursing experience preferably within Home Health specialty.
•Case Management experience preferred
•Valid driver’s license, car insurance and reliable transportation.
•Willingness to travel throughout assigned service area.
